The Vial of Life program provides emergency workers
with vital medical information about you when no one
else can.

Who should have a kit? Every
member of your family should have a kit.

How can I get a kit? Pick up
a Vial of Life kit from any Fire Station or rescue
crew. Details on how to use it are in the kit.

How much does it cost? There
is NO cost to residents. This is co-sponsored by the
Plantation Fire Department and the Westside Regional
Medical Center.
Vial of Life
Overview
The
Vial of Life program is an organized method of providing
paramedics with important medical information in an emergency
when others cannot. The Plantation Fire Department and Columbia
Westside Regional Medical Center have teamed up to provide
this life-saving service.
The
Vial of Life kit is provided by the Fire Department.
The kit consists of a health and medical information form,
vial and magnet.
1. You
simply fill out the form and place it in the vial. You may
complete the form by yourself or with your physician. Upon
request, a Rescue Unit will stop by to assist you fill out
the form and record vital signs as well as perform a routine
EKG. Additionally, you may choose to put other information
and sample pills in the vial.
2. The
vial is placed inside the refrigerator on the right-hand corner
of the top shelf.
3. The
magnet is placed on the refrigerator in plain view. The entire
process takes less than 15 minutes of your time. If you ever
have an emergency, that 15 minutes can save you a lifetime.
